Documenting assumptions before outcomes are known

Assumptions are easier to evaluate when they are recorded before an outcome is visible. A useful note explains why an assumption was considered reasonable and what evidence might challenge it.
Keep estimates separate from reported facts. Where a range is more appropriate than a single value, describe the variables that account for the difference.
This record supports a more constructive review. It makes it possible to learn from changes in the evidence without rewriting the original reasoning in hindsight.
